STUDENT required forms for Registration & Eligibility for SPRING 2024 BSM Athletics/Activities.

ALL BSM Students are requested to complete this eligibility and registration form for each season (fall, winter, spring).

SPRING 2024  list of current athletics/activities  may include the following --- Baseball, Boys & Girls Golf, Boys & Girls Lacrosse, Softball, Boys Tennis, Track & Field, Boys Volleyball, Clay Target, Fishing Club (Tournaments), Mountain Biking, Sailing (Wayzata Sailing Club), Ultimate Frisbee and the Option - Will NOT be participating in any of the above listed activities

  • **You will need to complete this form -- once for EACH SPRING activity you plan to participate in**
  • If this registration form is not completed -- student name will not be included on the spreadsheet submitted to coach(es) listing students who are interested in participating and status of paperwork.
  • Spring sports/activities have rolling start dates --- please check the webpage of the team/activity for start dates.
